PLEA

FolkStone

FolkStone is an alternative to institutional care for adults who, due to complex challenges and health conditions, cannot live on their own.

What we do...

We provide each individual with personal-care, home-care, nursing-care and medical-care. We also coordinate a wide variety of other services to support each individual’s health and well-being and to help them play an active role in the community they call home. These include dental, educational, physiotherapy, rehabilitative and occupational therapies plus recreational and vocational activities.

Why we do it...

Our aim is to provide each individual with a safe, nurturing home in a neighbourhood and community where they can live as comfortably and independently as possible.

Where we do it...

We have homes throughout the Lower Mainland and the Fraser Valley.

How we work…

Individuals live with one of our Family Caregivers who provide a safe, comfortable family environment.

More often they are certified Health Care Workers, Licensed Practical Nurses, Registered Psychiatric Nurses, Registered Nurses or have demonstrated strong skills and experience in providing health and social services.

Our staff teams have similar qualifications and are available 24 hours a day, 7 days a week in case of emergency.

We modify the homes, services and care we provide to meet the goals, strengths, needs, circumstances and preferences of each individual. Each individual is involved and in control of all decisions as much as possible.

Placements are typically long-term, but we also provide respite and shorter-term placements.

How to Access

Referrals come from a variety of sources including Vancouver Coastal Health Authority, Fraser Health Authority, Community Living BC, Department of Health & Social Services of the Government of the Northwest Territories, WorkSafe BC and Crime Victim Assistance Program.  To make a referral contact us at 604 561 3923.  Once a funding agreement is in place we accompany the individual and their family circle to view homes and assist them to move into the home of their choice.

Our partnership with Remedy’s Rx Pharmacy means each resident’s pharmacy needs are met.
