Assisted Living

Assisted Living is for adults (aged 19+) who can live independently and make decisions on their own behalf, but who require a supportive environment due to physical, functional and/or mental health challenges.

Our aim is to help each adult to lead a fulfilling life that is as comfortable and as independent as possible.

We have homes throughout the Lower Mainland and the Fraser Valley.

Our partnership with Remedy’s Rx Pharmacy means each resident’s pharmacy needs are met.

What we do…

We provide each adult with their own independent suite within the home of one of our family caregivers.  Program residents are also connected to a member of the PLEA team.

The resident, PLEA team member and Family Caregiver work together to create an individualized program of services.  This program of services may include:

  • personal care services which may include help with bathing, grooming, dressing, cooking and/or getting to and from appointments;
  • home care services which may include help with cleaning, grocery shopping, laundry and/or other housekeeping tasks;
  • medical care services which may include ordering, collecting and /or assisting with medications.
  • support to pursue personal goals which may include re-engaging in education, joining the workforce, learning a new life-skill, connecting to a community resource and/or engaging more in the community.

As this program is part of a full continuum of care, services can be stepped up or down as required.

How we work…

Our family caregivers are certified Health Care Workers, Licensed Practical Nurses, Registered Psychiatric Nurses and Registered Nurses, or they have demonstrated strong skills and experience in providing health and social services.

Our staff teams have similar qualifications and are available 24 hours a day, 7 days a week in case of emergency.

The services and care we provide are holistic and tailored to meet the individual goals, strengths, needs, circumstances and preferences of each person placed with us. Homes and families are selected based on their ability to meet the needs of the client.  Each individual is involved and in control of all decisions as much as possible.

Placements are typically long-term but we also provide respite and shorter-term placements.

How to Access

We accept referrals from professionals working for Community Living BC, Fraser Health Authority, Vancouver Coastal Health Authority and WorkSafe BC. We also accept private referrals.
