We help adults who would benefit from a short stay in a supported, family home environment as they transition from hospital care to a long-term placement.

Our aim is to support each client transition from hospital care to living as comfortably and as independently as possible in their community.

What we do…

We provide each individual (client) with an accessible and supportive place to stay while they consider their next move. While they are with us we work with them to coordinate their personal care, nursing care and medical care.

We also coordinate a wide variety of other services to support each individual’s health and well-being and to help them play an active role in their community. These include dental, educational, physiotherapy, rehabilitative and occupational therapies plus recreational and vocational activities.

How we work…

Our clients live with one of our Family Caregivers who provide a safe, comfortable family environment. More often our Family Caregivers are certified Health Care Workers, Licensed Practical Nurses, Registered Psychiatric Nurses, Registered Nurses or have demonstrated strong skills and experience in providing health and social services.

Our staff teams have similar qualifications and are available 24 hours a day, 7 days a week in case of emergency.

The services and care we provide is holistic and are tailored to meet the individual goals, strengths, needs, circumstances and preferences of each person placed with us. Homes and families are selected based on their ability to meet the needs of the client.

How to Access

A referral from Vancouver Coastal Health is required to access the program. To make a referral contact us at: 604 561 3923. Once an agreement is in place we assist the individual to move into the home. Once in the home we will support the individual to transition to a long-term home.
