Community Inclusion

Community Inclusion is for adults (aged 19+) who would benefit from some support in order to make their life as comfortable and as fulfilling as possible.

Why we do it...

Our aim is to help each adult to live a fulfilling life that is as comfortable and as independent as possible.

How we work...

We tailor services according to each adult’s individual strengths, needs, circumstances, preferences and goals. If these change, so will our services.

Where we do it...

We provide this service to adults living in the Lower Mainland and Fraser Valley. Adults who receive this service live in either our family care homes or independently in the community.

What we do…

We pair each adult with a one-to-one community inclusion worker who provides support and companionship, encourages them to identify and pursue their personal goals, and facilities the planning and actions needed to achieve them. These goals may include:

  • re-engaging in education,
  • joining the workforce,
  • learning a new life-skill,
  • connecting to a community resource, and
  • engaging more in the community.
How to Access

We accept referrals from professionals working for Community Living BC, Crime Victim & Assessment Program, Fraser Health Authority, Vancouver Coastal Health Authority and WorkSafe BC. We also accept private referrals. To make a referral, contact us on 604 561 3923.
